Step Two: Self-pay/ Insurance Verification

• If you are self pay proceed to the next step. (We provide a discount for payment at time of service!)

• If you have insurance confirm it is network. If you are able to find Lakeview Family Medicine or Glen E. Leer, D.O. on your find a provider search tool proceed to the next step (in network for most Medicare, commercial, and marketplace plans) feel free to call at 317-984-8811 if you have trouble finding us in your insurance directory listing and we would be glad to help!

Step Three: Health history or records (if any)

• Be prepared to provide to fill health history form prior to your first appointment

• Provide specialists records (if you have any)

  • Email records to Staff@lakeview.LLC  , fill out records request form when scheduling, or mail/bring in paper records
