New Buprenorphine Patients

Step Two: Review Self-Pay Costs

The buprenorphine program is open to new self-pay patients. An evaluation for buprenorphine treatment does not guarantee a prescription, prescriptions are only provided if deemed appropriate according to the doctor's medical decision making.

Program costs are below:

New patient deposit: $80.00  This must be paid prior to scheduling your first appointment. This deposit allows us to schedule extra time to go over as much as possible at your first visit. This is not refundable should you miss your appointment (you may call to reschedule ahead of time if needed).

Cost at first visit: $140.00

• Typically in the first month there are 3 additional weekly visits ($85.00) to allow for intimal stabilization to new treatment. (before moving to every 2 weeks).

• In the second month there is typically 1 additional visit  ($85.00) to continue to initialize stabilization to new treatment (before moving to every 3 weeks).

• Typically there is one three week appointment ($140) continue to initialize stabilization to new treatment before moving to monthly (every 4 weeks).

• Monthly Visits (every 4 weeks):  $140.00

Individuals needing additional stabilization may require more frequent visits than once per month. The above schedule is for a patient who demonstrates adequate stability to progress to less frequent visits (one visit per 4 weeks).

Additional costs:

Confirmation Drug Testing (can be billed to your insurance via the lab when performed)

Cost of medicine at pharmacy (can run through insurance at the pharmacy)

Testing performed outside the office via order

Services provided via an external provider

Step Three: Health history or records (if any)

• Be prepared to provide to fill health history form prior to your first appointment

• Provide specialists records (if you have any)

  • Email records to Staff@lakeview.LLC  , fill out records request form when scheduling, or mail/bring in paper records
